Sweep away: Minimally invasive biofilm removal

Abstract

Effective biofilm removal is fundamental to periodontal and implant therapy. Deep periodontal pockets, intrabony defects, extraction sockets, and complex implant surfaces can be challenging to clean thoroughly using mechanical methods alone.

To present a novel Er:YAG laser-induced photoacoustic approach for non-contact biofilm disruption and cleansing in periodontal and implant therapy.

Photoacoustic energy generates dynamic fluid movement and cavitation capable of disrupting biofilm without direct laser contact with the treated surface. Clinical applications include non-surgical periodontal therapy, surgical cleansing of intrabony defects, extraction socket management, cleansing of implant sites before implant placement, and treatment of peri-implantitis. The approach is designed to access complex anatomical areas while preserving surrounding soft tissues, root surfaces, bone, and implant surfaces.

Photoacoustic biofilm removal provides a versatile, non-contact adjunct for periodontal and implant site cleansing. Its ability to disrupt biofilm in difficult-to-access areas while supporting a tissue-preserving approach may expand the role of laser-assisted therapy in contemporary periodontal and implant care.

Learning objectives

  • Explain the principles of Er:YAG laser-induced photoacoustic biofilm disruption in periodontal and implant therapy.
  • Discuss the role of non-contact photoacoustic cleansing in managing biofilm within complex periodontal and peri-implant environments.
  • Identify clinical indications for photoacoustic cleansing in non-surgical and surgical periodontal therapy.
  • Demonstrate the clinical application of this approach in intrabony defects, extraction sockets, and implant sites prior to implant placement.
  • Describe its application in the management of peri-implantitis and biofilm-contaminated implant surfaces.
