Abstract

Bone resorption is a well-described phenomenon following teeth loss. It often induces horizontal and vertical resorption leading to an insufficient quantity of bone for dental implant placement. In the posterior edentulous maxilla, this resorption is often associated with the pneumatization of the maxillary sinus further complicating the clinical situation.

Ridge preservation and reconstruction techniques were introduced in order to palliate bone loss. Socket preservation by means of hard and soft tissue grafting along with immediate implant placement is the technique of choice after tooth extraction. This technique involves atraumatic tooth extraction and proper implant placement followed by socket grafting. Guided bone regeneration, ridge splitting and autogenous 3D bone grafts are often used in order to reconstruct the narrow ridges. Finally, maxillary sinus floor elevation is the technique of choice to palliate sinus pneumatization and reconstruct the posterior maxillary bone. These reconstruction surgeries involve delicate manipulation of both hard and soft tissues in order to avoid per and post-operative complications.

Although rotary instruments were usually used during bone surgery and sinus floor elevations, piezoelectric surgery currently represents the technique of choice for bone osteotomy, atraumatic extraction, implant site preparation and maxillary sinus window preparation and membrane elevation. Due to their selective action, ultrasonic instruments have proven to be very precise and efficient during osteotomy while being very conservative towards soft tissues thus reducing bone necrosis, sinus mucosal perforations and hemorrhagic complications. 

During this hands-on training course, we will explain challenges related to ridge preservation and reconstruction techniques and we will present the latest tips and advances in these implant site development techniques. The importance of new advances and technologies, such as radiographic planning, the use of piezosurgery and the proper choice of implant and biomaterials, will be exposed. These clinical advances will play a major role in reducing risks and complications related to ridge reconstruction procedures while preserving maximum clinical efficiency.
